Susceptibility weighted images of the brain at the levels of the midbrain (image 1) and corpus callosum (image 2) in a patient with a recent motor vehicle collision show small foci of hemorrhage (arrow), consistent with diffuse axonal injury. This type of injury is a shearing injury that can result in severe neurological impairment, typically occurring at gray-white matter junctions and the corpus callosum after rapid deceleration, due to the differing specific gravities of gray and white matter.](https://app.figure1.com/case-detail/3b623504-3e33-4111-961c-bd0752280a68)

Axial contrast enhanced CT (image 1) shows subdural collections of varying density. The collection on the left is hyperintense while the collection on the right (arrow) head is slightly hyperintense compared to adjacent cerebral spinal fluid. Coronal T2 weighted MRI (image 2) shows hemorrhagic material (arrow) overlying the left convexity. The subdural collection and CSF fluid surrounds both convexities. Note that the brain is atrophied with enlargement of the ventricles and prominence of the sulci. Diffusion weighted image (image 3) restricted diffusion related to ischemia within the posterior left parietal lobe. The imaging characteristics are concerning for nonaccidental trauma. Neurologic findings of nonaccidental trauma includes subdural collections of various ages, skull fractures, cerebral edema, and ischemia. Brain injury typically occurs due to shaking.](https://app.figure1.com/case-detail/76b51e1f-d3bb-4d1b-b986-24187be23b29)
