  1. Axial CT (image 1) shows a parenchymal hemorrhage (arrow) within the right parietal lobe. Subsequent axial contrast-enhanced CT (image 2 and 3) show innumerable blood vessels within the right parietal lobe. Digital subtraction angiography (Image 4) highlights the vascular anomaly consistent with cerebral proliferative angiopathy. Cerebral proliferative angiopathy is a vascular lesion with diffuse intravascular shunting. It is differentiated from an arteriovenous malformation by normal intervening brain parenchyma.

  2. The cerebral vasculature looking up from the base of the skull. These are the primary vessels, Vertebral arteries merge to form the Basilar artery. The carotid artery divides to give off the middle and anterior cerebral arteries. A keen knowledge of these vessels and the area they supply is essential.

  3. Axial and sagittal T2-W images show a large vessel near the straight sinus. Catheter angiogram injection of the internal carotid artery shows filling of a tangle of vessels anteriorly and early draining via the large midline vessel. Vein of Galen malformation is a heterogeneous term used to describe a cerebral AV fistula. This malformation can cause high output cardiac failure. #NeuroWednesday

  4. 14 yr old 3rd day post cardiac operation-ASD repair. Patient wakes up with right hemiparesis. CT scan, NON contrasted, shows “dense middle cerebral artery sign”. This indicates acute thrombus in artery. Subtle sign of hypodense left basal ganglia, due to oedema which reduces density on CT, indicates infarct developing in left cerebral hemisphere. 1st set of images show right basal ganglia outlined to compare with left side. Compare with unmarked image to the left. MRI scan then performed to assess for perfusion mismatch (images not shown) to decide if systemic anticoagulation worthwhile. Compare bright basal ganglia with the CT images. MRI angiogram shows cut off of left middle cerebral artery major branch (same position as thrombus seen on CT) and note decreased branch vessels over left MCA branches when compared to right.
