47 y old pt came suffer from cold sensitivity in upper right side , upon examination there are periodontal pockets and through x ray I found subgingival mesial caries of #17 ..note: no pain on percussion of upper and lower teeth

TT: first visit I did deep scaling of lower right quadrant and start to do drill mesial of #17 and go deeper and deeper and no caries appear !!! then I stopped and put IRM then did x ray to see where I stopped I found that still there is back spot beneath the IRM so I dismmised the pt and came after few days to complete , the pt was oky and the sensitivity gone then I replaced the IRM by vitrbond and composite after go deeper to remove the caries but no caries appeared then I did x ray no black spot beneath the filling and the filling perfect) and changed the old filling #15 because the old filling not good because no space between #15 and#16 no floss can go between so I changed it ..then I did deep scaling for upper right quadrant ..the problem is : the pt suffer again now from sensitivity like before ..the pt came again and I used the air and water to know which area cause the pain I found the pain between #15 #16 and 16 and 17..I removed the composite filling then I put IRM again to see!! what do u think !??

A female patient with age around 35 came to my dental clinic for filling. Although, the cavity was not deep she suffered from signs of irreversible pulpits, endo treatment was done. She continued to complain from pain till extraction is done. Then in another tooth with large filling this time there is a pain, with clinical investigation I found the filling is broken, I re-fill the tooth and I informed here of multiple pockets in her teeth as she complains from sensitivity and informed her that if the problem of sensitivity left untreated that will lead to irreversible pulpit is to her teeth that will need endo treatment. After that she came again assuming she had problem in the filling, I found that the tooth with filling is good although the presence of pocket, but the tooth after it has fractured disto-buccal cusp. The tooth has filling and there is no signs of caries. I informed her that this is because of the exposure of the sensitive parts of the tooth for long time which caused irreversible pulpits, she refused to do any dental procedure and looks she is not convinced with the diagnosis. My question is which part of my diagnosis is considered wrong medically?

A 21 years old female patient came to the clinic complaining from an intensive pain on hot and cold ( 7 of 1-10 scale) of upper left central incisor, she had a trauma before 10 years and clinically there is only a slight change in color of the same tooth. This is the periapical x- ray of the case. What is your opinion of the radiolucent lesion on the root of tooth 19 distally! knowing that the x ray was redone to exclude artifact and the image was exactly the same. It doesn't look like root caries also, does it?!

A new case came to after removing caries i found some thing look like guta percha cone base. After x ray i didnot find obturation in the canals , pts surprise me every day

female patient 34yrs came with pain on upper right area, after diagnosis found that there is periapical lesion related to upper right 7. root canal treatment was done with protaper system for palatal and distobuccal canals while mesiobuccal canal was calcified so I started to negotiate with k-10 and k-15 files until achieved the full working length. Irrigation used was sodium hypochloride 5% and lateral condensation technique is used to obturate the canals. follow-up x-ray will be posted after 1month.
