Severe Weakness of Unknown Etiology
A 55 year old man comes in with complaints of severe weakness, drooping of eyelids, double vision and aspiration. Patient’s wife states that her husband has been having progressive weakness for the last 3 months, but lately in the last week his weakness has become very severe. Patient has been following with multiple doctors, who have already checked him for Myasthenia Gravis and said he was negative. On referring EMR, records show a just-in positive AFB from a test performed several months back. But he was just checked for Tuberculosis and has been cleared. Specific organism hasn’t been identified yet.

A 12-year old boy living with HIV has been on treatment for extensive pelvic muscle myositis and both hips osteomyelitis for 6 weeks. 3 days ago he started having spastic lower limbs and hypertonia. He is afebrile with normal blood biochemical panel. LP was not done but his MRI is shown below. Any suggestions about DDx ?
68 y/o male presents with worsening SOB over the lasts several months. Upon 1V portable CXR, it can be seen why he has been short of breath. Pt has reportedly smokes cigarettes for the past 30+ years.
