AP (image 1) and internal rotation (image 2) radiographs of the shoulder in a baseball pitcher show widening of the proximal humeral physis (arrow), consistent with Little League shoulder. Little League shoulder is an overuse injury affecting overhead throwing athletes and is considered a Salter-Harris type I fracture equivalent. Treatment involves rest.
