14 year old obese female patient with pain in her left hip and knee for the last 6 months. Limb length discrepancy, external rotation of the left lower limb, limited abduction of the left hip. Full weight bearing is not affected. No Hx of trauma. Intraoperative image. What's the diagnosis and what procedure has been done?
