She is in septic shock with a multi-drug resistant (MDR) Pseudomonas bacteremia; she also has an unidentifiable yeast growing from both a sputum culture and a urine culture. In gathering a history, you learn the patient recently returned from visiting friends in India. While abroad, she was hospitalized for two weeks due to complications from diabetic gastroparesis. 40's male with a 2 month old wound to bottom of foot. Known Diabetic type 2. Today has fever, tachycardia and pus draining from foot and sever pain. Poorly controlled blood glucose. Lactate =2.9, Wbc 25k, heart rate 130-140 with a fever. Treated with Zosyn and vanco. Podiatry/ortho to consult. Do you think this foot will be salvaged? No gas seen on X-ray however severe osteo is suspected. Further imaging, blood cultures and consults are pending.
