A 64-year old architect complains of left handed tremor at rest, which interferes with his writing and drawing which started 3 years ago. He also notes a stooped posture, and slight unsteadiness on turning. He remains independent in all activities of daily living. Physical and Neurological Examination: Flat facies, monotonous voice, a rest tremor of the left arm and leg, mild rigidity in all limbs and impaired rapid alternating movements of the left limbs.
What is the likely diagnosis and prognosis?
