38 year old with this, which was present last year. Denies traumatizing the nails repeatedly with any tics or OCD. Was treated last year with a course of Ketoconazole 2% cream with no effect. Iron level has been ordered. Chronic bacterial paronychia is suspected. How would you treat this?
