Called to private residence for a DK party.

46M found lying on the floor of his living room, incontinent of urine and altered. Pt’s boyfriend states that the Pt is a chronic alcoholic and was drinking all day yesterday. Pt stopped drinking around 6pm and laid down on the floor. Boyfriend found the pt still altered on the floor in the morning. Pt has a hx of ETOH withdrawal and presents as such. Pt very tremulous, non verbal, and incontinent. Pt also had oral trauma from a suspected seizure. No other hx provided.

Vitals: 168/100, HR 110, BGL WNL, O2 WNL. Besides oral trauma, no other external signs of trauma noted. Pt’s skin was cool, pale, and diaphoretic. Pt presented w/ full body tremors and tongue fasciculations. Pt also vomited during transport.

Hospital started Pt on phenobarbital treatment. 3 hrs later Pt defecated on himself and wasn’t getting any better, was sent to CT scan. Thoughts?
