pt is a 72 y/o male. Patient awoke at 0200 hrs with chest pain which increased upon movement and progressed to 10/10. Patient presents standing up in his yard outside the residence. Patient is cool, pale, diaphoretic. Pt has no medical history of any kind and does not take any medications daily. Pt movement is stopped. Could not transmit 12-lead due to poor cell service. 324mg ASA administered, 0.4mg NTG (2) administered to no relief. OLMD contacted, 12-lead explained in great detail to ED physician; I requested cath lab activation. ED physician refused and advised me to transport non-emergency or emergency at my discretion. I transported emergency to the receiving facility approx. 30 mins away. Upon arrival, pt presents to ED physician without change in initial presentation.

74yo African American male with history of HIV, Hep-C brought in by daughter after days of no appetite and persistent weight loss. Pt denied any symptoms, except for a "slight pain across his upper chest" before he came in. Pt was pain-free upon arrival. EKG obtained, and cardiologist was consulted. Cardio wasn’t concerned since patient was pain-free. Troponin 0.108 ng/mL. Other labs within normal limits. Hourly ECG’s were obtained and no changes were noted. Patient remained pain-free during ER stay, and ended up being admitted for pneumonia and further chest pain workup. What is causing this STEMI-like pattern? 51 y/o M c/o CP while shovelling in backyard. Feels like a block is sitting on chest. Hx of #Hyperlipidemia, no other cardiac hx. Treated with nitro Asa morphine with minimal relief. Vitals hr 60, BP 140/70 rr 20 mildly laboured diaphoretic ++. The closest cath lab refused direct acceptance due to backup of pts. Next closest cath lab 25 min+away which will accept. Question: where do you go with the pt closest cath labs ER? or delay to further cath lab 25 min away? For students please tell me what you see on the ECG?

The patient presented with GCS 15, AOx4. Patent airway with oxygen saturation greater than 96% on 2LNC. EKG shows wide complex tachycardia with rates 160-170. Manual SBP 80s/40s. Fast ultrasound was negative for free fluid in the pericardial space. CXR performed. No sign of pneumothorax, with small apical hemothorax. Blood administration initiated due to hypotension, tachycardia, and potential for internal blood loss with mechanism of injury prior to CT scan. Cardiothoracic surgeon consulted, no surgical intervention at this time given the risks and benefits. Admission for further stabilization and monitoring.

patient in late teens presenting after a collapse, no chest pain, no previous cardiac history, no history of sudden death in family, are there any features in this ekg that would warrant further work-up or is this just a pediatric ekg?
