Case Presentation
This 38 year old man presents to the ER with a one day onset of erythema to the thenar surface of his palm. Patient noticed the “red line” at 10 AM. Within 6 hours his Palm was burning and itching. 6 hours after that he noticed red lines on the MCP joints of the second and third digits. A few hours later these areas became darkened. Patient denies pain out of proportion to presentation. It “burns and itches”. If you notice a scar on the wrist it occurred 3 weeks prior and was not infected at the time. Oh, the scar is the result of a rabbit 🐇. 😃

Patient in my rehab facility presents with reddened and itchy palms (without any sort of lesion here) plus swelling and itching of his right wrist (flat, raised bumps on the wrist, hard to see here). He has been putting lotrimin on it for several days and it was relieving the itching, but the redness has spread and it continues to itch. I think it might be your run-of-the-mill fungal infection, but would this also explain the swelling?
23 years old female presented to my clinic with intense itching on B/L upper (palms included) and lower limbs since 2 days. Patient will have itching on Normal skin and after itching the above erythematous lesions will develop. No h/o atopy, fever, insect bite, contact with allergen or any drug intake. No family history of similar complaints. The differentials and line of management.
64-year-old male patient who came in with pain, burning, itching and inflammation in the right eye, without loss of vision. History of stroke in the left eye, no history of diabetes, hypertension or glaucoma. Initially, he washed his eye after shaving and 24 hours later he noticed minimal discomfort and the presence of eye discharge (moment 1). He then decided to clean his eye with a cotton swab and chamomile infusion, which he uses occasionally (moment 2). 12 hours later he reported pain, inflammation and the following image. The treatment was betamethasone + tobramycin every 4-6 hours. We are unsure whether the cause was at moment 1 (there was no foreign body noticeable at any time) or at moment 2 due to some component of the makeup remover or the infusion. Any ideas regarding the etiology?
