Update
Pt remained unstable in regards to BP throughout the night (on multiple pressors BP monitor through art line) pt arrested in ICU in early am with ROSC within 3 minutes, pt would withdraw to pain but no other responses, pt transported out to larger hospital to be placed on ECMO.

HTN pt present with severe headache Bp 180/100
76yof complains of gradual onset or chest pain. Pt stated it began as left shoulder pain and spread to the full length of her left arm and into her chest. Pt rates pain 9/10. Initial BP 180/90. Stemi protocol activated immediately. Pt give three 0.4mg ntg sublingual with immediate relief. Pt BP never dropped below 180 systolic. Pt taken immediately to cath lab upon arrival to hospital.
30yo/m came to er for palpitations otherwise asymptomatic. received 6&12 adenosine in er no change. Pt came to floor on cardizem gtt hr 186 bp 105/80. Card d/c'ed gave 5mg lopressor iv push no change in hr. Bp dropped 82/57. Amio started. Bp dropped to 44/36 pt lost consciousness. 1 shock delivered pt converted to nsr bp 90/70 sent to icu. What is this rhythm?
46 yo male admitted for cataract surgery (under LA) & pre-op work up. Known case of DM (controlled) on OHA. No other complaints, not known hypertensive. Pulse rate and rhythm normal. BP was recorded high on several occasions hence started on amlodipine 5mg. #1 ECG is the one done during pre op investigations. After his BP was under control, taken up for Sx. As pt reached OT, BP shooted up to 160/100 along with ECG changes as in #2. Certain measures were taken (I'm not aware of) but Sx was cancelled. Then pt was started with telmisartan and hydrochlorthiazide. BP was still fluctuating b/w normal and bit higher range but 2 days after the incident, pt again started having irregularly irregular pulse rhythm. Throughout all this, pt had no symptoms at all.

patient in late teens presenting after a collapse, no chest pain, no previous cardiac history, no history of sudden death in family, are there any features in this ekg that would warrant further work-up or is this just a pediatric ekg?
