Patient presentation

Patient presents with extreme throat pain for the past three days. Slightly hypovolemic with body aches, high fever, no signs of external rash.

Question for discussion

What would be your presumptive diagnosis and medication recommendations?

• 24yo male. 102F (38.9c) and swollen neck lymphs. History of strep throat 2-3/year. Current tx of augmentin + chlorhexidine mouth wash. Candidate for #Tonsillectomy?

• 5 day progression of Strep Pharyngitis (rapid +) with progressive uvulitis. 30yr old female. No HX of frequent strep infections. Non smoker. No meds. No comorbidities. Day 1- 104 fever with body aches. Started amoxicillin. Day 3 - Fever subsided, no improvement in throat. Uvula swelling and sitting on top of tongue causing choking sensation. Oral prednisone started. Day 5- some improvement in tonsils but uvula ulcerating. ABX changed to augmentin and culture collected - pending at this time. Viscous lidocaine ineffective for pain associated with the uvula.

• 38 YO male with three day onset of sore throat and urticaria rash on trunk. Strep and mono are negative. What is it and what treatment do you prescribe? (Yeah I know it's easy, but not everything is a zebra 😃)
