35 year old male patient, as per him, he was treated for keloids 3 times but no effect. he has extensive keloids on bilateral shoulders, anterior chest, abdomen, lower back and anterior shoulders. He has multiple nevus which according to him has been present since birth. I think they are cafe au lait spots and keloids could be due to correction of neurofibromatosis lesions in the past. What would be your differential and how would you approach this case?

A subtle presentation of an interesting condition. Adult male with a patch of skin containing numerous neurofibromas. Represents segmental or mosaic #Neurofibromatosis, where there is a post-conception or somatic mutation causing NF. Transmission to his children can only happen if the mutation is in also in his gonads.

70 yo WF with 20y hx HTN lost 21 lbs on a semaglutide but still BMI over 30, shows drop in BP when standing, stabilizing at 102-116 systolic, 65-75 diastolic. Discontinued all HTN meds x 30 days and no episodes of HTN. Could a small weight loss like this cause a drop to normal BP? Has anyone heard of a side effect of lowering BP?
