Case Summary: 35 yrs, female. HIV positive on ART since 2014. Presented with productive cough associated with pleuritic chest pains & progressive shortness of breath x 2 months. Accepts paroxysmal nocturnal dyspnea, night sweats but no fevers & weight loss. No previous history of TB, HTN, DM. On 2 May 18, switched from TDF/3TC/EFv to AZT/3TC/ATZ/r due to ART failure. O/E mildly distressed, puffy face, JVP not raised, PR 120 b/min, RR 24 c/min, Temp+BP normal. Normal heart sounds; respiratory exam significant for bilateral crackles. Notable bilateral pitting edema of lower extremities. FBC review showed HB of 4.6, Plt 94, normal MCV, Urea 257, Cr 9.2, Sputum Neg AFBs. Waiting for gene Xpert results for sputum. Viral load: 156740 copies.
