Patient Case

Elder male presents with pruritic whole body rash (scalp, chest, arms, hands, penile head, upper thighs, lower legs) x 2 weeks worsening.

Treated initially on his own with a bleach water bath and Benadryl and topical medicine honey but worsened. Subsequently went to urgent care and diagnosed with possible scabies and infection from scratching, so was given doxycycline and permethrin treatment and a steroid taper. Symptoms briefly started to improve but suddenly worsened again. Went to dermatology and prescribed triamcinolone topically but still no diagnosis given, no labs, no biopsy. His was 1 day ago and now seeing me. I’m thinking fungal vs psoriasis but also ordered (CBC, Ana, seed rate and RPR). Any thoughts?
