The uterine cavity (arrow) is visible to the level of the cervical os. Teaching point Cervical adenocarcinoma arises from glandular epithelium of the cervix and typically presents as a bulky endocervical mass on MRI. Preservation of the uterine cavity with extension through the cervical os helps localize the tumor origin to the cervix rather than the endometrium. This diagnosis is rare in adolescents. In this age group, the imaging differential includes embryonal rhabdomyosarcoma, which can have overlapping features. Imaging alone cannot reliably distinguish these entities, and histopathologic evaluation is required for definitive diagnosis.
