Patient with mild cognitive impairment instructed to draw a clock "set to ten after eleven" (image 1) and copying the same clock (image 2). Attempted to compensate for tendency to forget instructions by writing 11 10 in the top right of the paper, but later misinterpreted their own reminder as only needing to label 10 and 11.
74M presents to ER via ambulance accomp. by son. Found on floor by son who states that pt admitted to son he fell and stayed on the floor X 1d. On admission, pt noted to be responsive but lethargic. No notable abrasions or lacs. VSS but hypothermic (T33.1°C) w/ euglycemic. GCS 10-11. Labs indicated multi organ system failure. During intubation, suctioned vomitus. Initiated Tazocin. Attempted to stabilize pt but became hypotensive/tachycardic in afib w/ RVR. PMHx: mechanical valve repl. (Coumadin). INR 4.5; reversed w/ beriplex. Initiated Levophed. Stat transfer to neuro institute. Great case!

patient in late teens presenting after a collapse, no chest pain, no previous cardiac history, no history of sudden death in family, are there any features in this ekg that would warrant further work-up or is this just a pediatric ekg?
