Patient Case: 14 years old female

Chief Complaint: 14 years old female complains of pain in her big toe.

Medical History: She doesn’t have any medical problems except for atopic dermatitis and eczema which she uses steroids on her arm for. No history of hypertension or diabetes. No previous operations or surgeries.

Similar Case: 53 Years old female with normal BMI diagnosed with Type 2 DM. Her HbA1c was found to be 11%. Advised on dietary modifications, exercise, and prescribed Metformin 250mg BD; increased to 500mg BD. Presenting complaints include paresthesia at night without other symptoms. BP and pulse normal.

Patient Case: 44 yo female

Presentation: c/o “burning pain” radiating from spine (approx C5-T1) to her breast. Pain consistent with shingles but no rash present. Querying if shingles can present without a rash.

Patient Case: 27 yr old female

History: Labeled as bronchial asthma for 9 years. Started on omalizumab, showing improvement and decreased ER visits. Admitted with spontaneous urticaria, chest wheezing, treated with diphenhydramine and hydrocortisone. All allergen work-up negative.

Patient Case: 52-year-old man

History: Presented with 20-minute history of chest pain radiating to the left shoulder and neck, associated with dyspnea and diaphoresis. History of hypertension, smoking, and sedentary lifestyle. Upon examination, bibasilar crackles noted; referred to the emergency department.

Patient Case: 28-year-old woman

Presentation: Intense pruritus over the lower back for 3 days, no recent travel or exposure to new environments. Results from physical examination revealed multiple small vesicles predominantly in lumbar region and left gluteal area.

Patient Case: 4-month-old male infant

Presentation: Skin lesions localized to the chin for the past 3 days. Multiple small pustules with inflammatory signs observed.

Patient Case: late teens

Presentation: Patient in late teens with no chest pain or previous cardiac history, collapsed. Questioning if EKG shows anything warranting further work-up.
