Patient 60 years male presented with Right Inguino Scrotal swelling, Abdominal Pain and Distention, constipation, vomiting loss of appetite since 01 week. One week back he had c/o dysentry 1 episode and has taken some capsule since that day patient presented with these symptoms. Patient also gives Hx of Hydrocoele since age 18 years, didn't got operated but aspirated multiple times at monthly intervals. No other comorbids. U/S shows right inguinal hernia with testes displaced, marked dilated loops.
