Case Update

Update to the 40 year old male with the infected Achilles tendon repair 4 weeks postop. I took him for an I&D. I removed all the sutures I could find and left the swivelocks in the calcaneus. Intraop cultures were taken. Interestingly, his Achilles had healed a nice bridge of tissue over his rupture (image above-prone with heel at the bottom) I approximated the wound and put on an incisional wound vac.

Cultures isolated MSSA so he was treated on keflex.

Once his wound healed, I put him back on the normal rehab protocol.

He has returned to full activity.

When I saw these, I called him and he was shocked at my idea of going to the ER. Then they put him in the Intensive Care Unit. No melana or bloody stools. What is the lowest walking Hemoglobin you have seen? Work up in progress.

update on 4 year old. Wounds are healing by using a medical honey scrub which seems to be working well. Child has had a negative blood culture for 4 days. Would anthrax show up on a C/S from the wound? The biopsy and wound culture were negative . The child is currently still taking Cephalexin for redness around the wound. I strongly doubt this is a brown recluse bite because he's a very talkative 4 year old that I would assume would've felt 2 bites and would've had symptoms. He's had zero pain or itching. So does anyone else have an idea what it could this be?
