Healthy, 37 year old male. Started having these semicircular lesions on his tongue approx. 7 mo ago. Main complaint is a burning pain. Occur roughly every two weeks. My thought was that these were apthous ulcers, but I thought they were generally sporadic, rather than reoccurring as in this case. He has seen a number of specialties with negative serologies, otherwise normal lab work. Thoughts?

He had a history of hypertension, long-term smoking, and a sedentary lifestyle. On physical examination, bibasilar crackles were noted on lung auscultation. An electrocardiogram (ECG) was performed in the primary care setting, and the patient was promptly referred to the emergency department.
