Case Overview
58yof, underweight, heavy smoker, lost her business now in dire financial straits, "from doctor to doctor" getting more pills. I counted 4 different benzodiazepines, 3 different antidepressants. Says she does not take them all, but admits smoking and drinking alcohol heavily. Then comes out another bag... "and this"? "Oh no, that's not medicine, that is all "Natural stuff" from "health shops" or herbalists."
Drinking lots of water, I clinically suspected hyponatraemia and am in the fortunate position to have an "Opti-Lion" cassette electrolyte analyzer in my office... note the hyponatraemia and alkalosis.
And as for "Harmless, Natural" stuff, well Poison Ivy, Arsenic and Snake Venom are all totally natural. In my book if it looks like a pill it is a pill. Lots of processing to get it to look like that, lots of interaction with real pharmaceuticals but not dependably the same concentration and effect in each batch.

Case 3
One of the most emaciated human beings I have ever treated. 20-something female, heroin and no food for heaven knows how long. Came in c/o withdrawal, when we removed clothing all of her abscesses began to rupture as they were dried to her clothing. Pt tachypnic and in distress. Pt immediately intubated, and central line placed. As you can see from last photo we removed over 100ml of purulent drainage from wounds. Pt did great and actually walked out of the hospital a few weeks later. Wish we could show teenagers stuff like this - drugs are just terrible.
