67 year old patient with medical history of hypertension (TX since 10 years ago, non adherent to tx with enalapril). She previously had surgery for her varicose veins on the right foot (unknown to her what type of surgery/procedure). She came because of a chronic back pain with insidious onset since 1 year ago + chronic pain in both of her knees.

On physical examination:

Extremities as shown on pictures. Intermittent claudication, paresthesia on right foot, pain to touch on both foot and leg, dry atrophic, bluish skin discoloration on right leg, popliteal pulse non palpable.

Female, 81 years old. Seven days ago she begins pruritus on her right leg and sensation of local burning. An erythema appeared on the lower right leg area, which is increasing in size. In the evaluation there is presence of dilated veins in the medial face of right foot and ocher dermatitis in the ipsilateral leg. What is the probable diagnosis? What’s the Treatment?

Female patient 38 years came with the incapacity to move both of her legs and below her belly button. Also, thermo-algic sensation has disappeared. This is her left leg, with this skin condition that appeared after two days. She told that she had the same skin condition 2 years ago in the upper gastric area. Since 8 years she has fever and periods of remission without treatment. She has a little appetite, she's exhausted and has lost strength. No other medical exploration was done, due to lack of resources. She came to the cabinet of Dr. Taghy, neurosurgeon with whom I'm currently in summer practice. She had a surgery for an ovarian kyst one year ago, and had also an unknown operation for the vessels of her left leg 8 months ago.

A 52-year-old man presented with a 20-minute history of chest pain radiating to the left shoulder and neck, associated with dyspnea and diaphoresis. He had a history of hypertension, long-term smoking, and a sedentary lifestyle. On physical examination, bibasilar crackles were noted on lung auscultation.
