My diagnosis was multiforme erythema and I treated her with cetirizin and dermocorticoids. She went to ER this WE because of pain and had a blood sample which was normal. Today (picture upper), she confessed having put some argile on her shoulder few days ago because of articular pain... I will wait and make a biopsy if the things don't get well in 3 days (if she agrees)

Patient was recently started on Welbutrin and Neurontin for "something going on with her neck". She developed this rash a few days later and was then seen at an urgent care and given Benadryl and Pepcid. When I saw her I took a punch bx and ordered a CBC w/ diff and an UA. I prescribed her a prednisone taper and fluocinonide cream. What are your differentials?



36 years old women she suffer from redness in her face , and little in body
Before 7 years the condition started ,, she saw that her face begin to be red and little swell as in picture that i uploaded ,
When she take steriod or antihistamine the result was very good but when she stop taking these medication after 7 days her face back to reddness as in picture
Note: there is no itching or pain
What is the diagnosis and what is the treatment??
She is not DM
Not hypertension
And dont have chronic disease
No smoker



A 65-year old woman referred to me because of 2 day skin change on the inner side of the left knee. She was working in the field, and after she came home she noticed redness and two punctiform lesions in the middle in mentioned area. During next day, the redness progressed, and she now feeling pain in the region with skin change. There is no fever other symptoms.
On exam, I noticed smaller livid redness in the middle, which is indurated and raised from surrounding skin with small pit in center, and bright redness around, which blanch with preasure. The whole area is warm on touch, and painful. There is no lymphadenopathy. The rest of exam is normal.
My thinking is that this is most likely some reaction to some insect bite. Infection is also possibility. Despite its look, because of symptoms and quick progression of lession, I would not consider erythema migrans.
What is your opinion?
P. S. The brown spot on the picture is some stain on my camera, not on the skin of the patient.



This is a follow-up. Apparently, patient omitted to tell me she has inserted her hands in a bucket filled with water in her garden. Mosquitoes larvae were noticed when she brought the water sample to ambulatory care. She presented next day after seeing me to A&E because the hands worsened and she was commenced on oral steroids and oral antibiotics and discharged home same day. This is how the hands looks like 5 days after this treatment.

patient in late teens presenting after a collapse, no chest pain, no previous cardiac history, no history of sudden death in family, are there any features in this ekg that would warrant further work-up or is this just a pediatric ekg?
