23 yo Female. Has maculopapular rash which will blister on bilateral elbows, hands, fingers, palms, feet.. Blisters are filled with clear fluid. Rash is pruritic and present for four months. Biopsy of lesion is negative for cancer or scabies. No other complaints except a lot of bruising on arms and legs. She denies any pain, but has been having headaches which might not be related. Only medication is birth control pill which she has been on for 3 years.

55yo male. 3 year history of this very painful itchy rash on soles of feet mainly arches. Cannot wear shoes or use wet dressings as apparently makes it worse. Has trialled topical steroids / vitamin D analog / oral steroids (prednisone) / antibiotics/ now on methotrexate for 3 weeks and no improvement yet. Has needed opiates at times for the pain which is worse at night and prevents sleep. He thinks it started when he brought a new pair of sandals as the rash came on straight after but has now persisted in a cycle for last 3 years. I was thinking that it looked like Pompholyx? Apparently all skin biopsies / bacterial / fungal / viral swabs all have been normal or inconclusive. Thoughts please.

50 year old female with history of diverticulitis, asthma, gerd, eczema and c-diff presents with new onset of rash on only the palms of hands after taking medrol dose pack for dermatitis flare on her arms. Feet pics are chronic but increased pruritus. Any idea what the rash on her hands could be? She has tried on her hands and feet...betamethasone oint, urea 40%, clobatasol cream and miconazole powder without much improvement. most recently added terbinafine cream. Hands are very itchy. Thank you for any input.

A 45 yo F with a PMH of TTP presented with a rash on her forehead that has been present for over a month that has slowly progressed into a larger rash. She denies pain, pruritus, or new lotions on this area. When asked about hair products, soaps, dyes, and any new creams she is using she does not identify any new ones but she is unsure if her hairdresser changed products on her. She uses dove and another non scented soap which are not new. She did try some triamcinolone cream by another provider and she reports “it made it drier” and thus she stopped this. No other rashes including flexeral areas. She denies fever, joint pain, Malay rash, mouth ulcers, rash behind the ears. No other rashes observed on exam. I prescribed lotrisone cream. Thoughts?
