A patient with recently diagnosed limited ANCA vasculitis presented with a worsening necrotic skin ulcer. She had been on methotrexate for sino-nasal disease but this lesion - which started "like a pimple" has been expanding over the past 2 months. Biopsy was significant for a neutrophillic dermatosis and consistent with a diagnosis of pyoderma gangrenosum. Her therapy was escalated to rituximab and she improved steadily over the coming months.
