2024 Friday Quiz 5 Answer

Sagittal (image 1), coronal (image 2), and axial (image 3) T2 weighted images as well as axial post contrast (image 4) and diffusion weighted image (image 5) show the tectal mass. Overall, the mass is mostly solid with small T2 hyperintense foci, mild heterogeneous enhancement, and restricted diffusion. Atypical teratoid/rhabdoid tumors (ATRT) typically present in young children. The lesions may mimic medulloblastoma but can occur in other locations within the brain. On imaging, they are often enlarged and heterogeneous. The tumor is typically caused by inactivation of the INI/SMARCB1 gene. ATRT tumors are associated with rhabdoid tumors of the kidney or liver. Thus, abdominal ultrasound is often performed after ATRT diagnosis. Other tumor types associated with INI/SMARCB1 inactivation include epithelioid sarcomas, chordomas, and renal medullary carcinoma.

Axial CT of the head (image 1) shows a partially calcified mass (arrow) bordering the atrium of the left lateral ventricle. Axial FLAIR (image 2), T1-weighted postcontrast (image 3), and axial T2 weighted (image 4) MRI show the heterogeneous partially calcified and enhancing tumor (arrow) centered at the junction of the left cingulate and hippocampal gyri. At resection, this was found to be a ganglioglioma. Gangliogliomas are low grade tumors with a heterogeneous mixed cystic and solid appearance. BRAF mutations are present in up to 60% of patients. This mutation historically has led to portions of rival for pediatric patients with a low-grade glioma.

Axial (image 1) and coronal (image 2) T1-weighted, axial T2 weighted (image 3), and axial T1-weighted postcontrast (image 4) MRI show the lobulated mass arising from the glomus of the left choroid plexus. The mass diffusely enhances. Choroid plexus papilloma and choroid plexus carcinoma are the most common choroid plexus tumors. They are most commonly diagnosed in children younger than 5 years of age. Papillomas cannot be reliably distinguished from carcinoma. However, the homogeneous enhancement pattern is more common in patients with papilloma.

Axial T2 weighted (image 1 and 2), T1-weighted (image 3), diffusion weighted (image 4), and T1-weighted postcontrast (image 5) MRI shows a large mass arising within the left liver. After biopsy, the mass was confirmed to represent diffuse large B-cell lymphoma. It is difficult to differentiate hepatic lymphoma from other hepatic tumors. Primary hepatic lymphoma is rare. Diffuse large B-cell lymphoma can affect the liver and is more common.

Axial CT (image 1), axial T2-weighted (image 2), axial T1-weighted (image 3), and axial T1-weighted postcontrast (image 4) MRI shows a large heterogeneous tumor involving the left posterior parietal lobe. The tumor was diagnosed as a glioneuronal tumor. Glioneuronal tumors are heterogeneous with solid and cystic areas. Calcification and hemorrhage are common.
