Lesion under the belly

43 year old male with Type 2 diabetes not well controlled (A1c 7.5%) presents to the clinic with painful lesions to the touch under his stomach and crotch. Previous doctor treated it with ketoconazole 2% and oral fluconazole. Patient states he would see the lesions disappear then come back after a month. This has been ongoing for 8 months. What would be the best treatment option for this patient?
