32 year old, had similar rash on his leg twice in last year, disappeared in 2-3 days without any treatment.
68 yr old male found by mother outside in chair unconscious difficulty breathing. Upon first responder arrival found pulseless & apneic, CPR started immediately, AED advises two shocks. When I arrived he was asystolic. 3 rounds CPR and Epi given, intubated and achieved ROSC. These are the serial 12 leads obtained during our 20 minute transport to a PCI center. BP remained in 70s systolic even after 1L bolus. Administered 7.5mcg/min Levophed w/ no change. Increased to 15mcg/min which brought BP to 120s systolic on arrival to PCI center. Pupils round, equal, reactive, spontaneous resp, and purposeful movement at turnover to ED. Brought straight to lab had a LAD occlusion and now in CCU. My Paramedic student, first responders, EMTs and I might actually have a true Code Save!!! So proud of my Paramedic student for doing a PHENOMENAL job!!!
Male, 17 year old without any complaints, except slight abdominal pain. No have possibility to make CT. Any ideas?
24 y/o female c/o vaginal discharge and "possible exposure to STD." States she was previously tested in ED 1 week ago for vaginal itching and discharge and called to advise results were abnormal and to F/U with PCP but does not know results. States that incidentally has had rash for the past month that started on feet and has spread to legs, and is now starting to notice rash on back and proximal arms. Denies F/C, or any recent illness. Rash is not pruritic. Small red papules noted on BLE, proximal BUE, and lower back. Each papule has surrounding white halo approx 1cm. No palms or soles involvement. Any suggestions as to what rash could be?
