63 yo Caucasian smoker presents to UC complaining of SOB, productive cough, wheezing x 2 days. Pt was seen a week and a half prior with O2 of 83% ORA Pt was subsequently sent to ER via ambulance and admitted for 5 days. Pt was discharged only supposedly only given 1 day of outpatient abx and 2 of steroids. Pt is a known smoker with Asthma and still smoking during symptoms. Vitals: O2 93% before duo-neb and 95% after. BP: 122/72 Pulse: 64 Resp: 16 Temp: 98.2. Pt in no distress on exam with diffuse expiratory wheezing and diffuse rhomboid over all kind fields. Exam otherwise normal. Pt currently on Albuterol, ipratropium, Symbicort. Dx and tmt?
