68yo female presenting w/ epigastric discomfort. PMHx Diverticulitis, Osteoporosis. Seen by ER who noted pt to be looking pale and "unwell". Placed in monitors (pic#1). Bedside echo and CT chest also confirmed a stable pericardial effusion. Initially treatment of cardizem IV given with no initial effect but converted after 4 mins of infusion of procainamide (pic#2 & 3). Regular monitoring in pic#4, but last night reverted back to tachyarrhythmia state, cardizem given with similar pause, converted with sync cardio. (200J biphasic). Pic#5 post sync cardio.
