Case Summary
Patient was started on scabies treatment topical. No change. She was given topical steroid for the lesions. Says the scabies treatment improved it but after 3 days of treatment the lesions became pruritic again. My thinking is it's either atopic dermatitis or a complicated case of scabies. Rare thing might be reactive perforating collagenosis. Thoughts on differential diagnosis?

New patient reporting this rash for months that hasn't responded to topical/oral anti-fungals or topical steroids.
Intensely pruritic with no systemic symptoms. Lesions on palms as well. Ideas?

#Onychomycosis in prisoner patient.
Treatment of onychomycosis depends on the clinical type of the disease, the number of affected nails, and severity of nail involvement. A systemic treatment approach is better. Oral AND topical antifungal treatment + antibiotics.
