77yo male presenting to ER post-cardiac arrest; received shock from ICD, had ROSC on arrival. PMHx: DMII, HTN, CABG (quadruple), BPH. EKG#1 (initial EKG on arrival, EKG#2 60min later. ER-MD consulted cardio on sight and refused to sent to cathlab. Pt was initiated due instability, and admitted to ICU. Trop + 67, 87, 319, 747. EKG#3 today. What should be done with this pt? What could have been done? What would you guys have done? What rhythm(s) do you see?
